In addition to office visits, he follows eligible patients in hospitals, rehabilitation facilities, skilled nursing facilities, and through house calls when appropriate, coordinating with specialists, healthcare teams, home care providers, and families.

Education & Training

Dr. Fuzaylov is board-certified in Internal Medicine and completed his residency training at Flushing Hospital Medical Center in New York after postgraduate training at the Hospital of Saint Raphael, affiliated with Yale University School of Medicine, and New York University Hospital. Prior to coming to the United States, he completed medical school at Tajik Medical University in Tajikistan, followed by neurology training. He practiced as a neurologist before relocating to the United States, bringing a broad and diverse medical background to the care of his patients.
